Brook and Meadows is an apartment style Rehabilitative Community Residential Setting providing daily supervision in a structured setting. The individual is generally accommodating to staff requests, medical care and medication management. The individual may still require basic medical needs, such as administration of medication and coordination of appointments and/or transportation. Most individuals will be on different stages of a self-medication management program. The individual may have only one to two direct verbal threats to peers or staff in the past two to three months. The individual may have had one to two physical aggressions towards objects or property in the past two to three months. The individual may have outside activities such as day programs, jobs, or school. Chemical and mental health symptoms typically are not major concerns, but may cycle or flare up one to two times every two months. Individuals with this program will be building more independent skill sets related to apartment living and beginning to explore community options out of a Community Residential Setting model.
